ActualMeds' Pharmacist On Demand Service assembles and validates data from multiple sources, including admission, discharge, and transfer notifications, as well as "historic" home medications with acute care discharge orders in PointClickCare and data from pharmacy dispensing systems. The first-of-its-kind integration promotes successful transitions of care seamlessly, from admission to discharge, across the continuum while supporting compliance with Minimum Data Set (MDS 3.0) requirements.

It is currently available in the PointClickCare Marketplace and can be combined with Medication Management Reviews (MMRs) or complement existing consultant pharmacist services.
