Connecting Skilled Nursing Facilities (SNFs) to the care continuum

Our POD team performs real-time best practice medication reconciliation upon admission to the facility, comparing a patient's "at-home" medication with their hospital discharge medication. This process helps to avoid therapeutic duplication or omission of a needed drug, reducing the risk of adverse drug events that could lead to re-admission to acute care. Additionally, PODs provides medication reconciliation when a patient is discharged from a SNF to home, optimizing their regimen and ensuring proper understanding of medication use. SNFs that demonstrate these best practices and reduced readmission rates can become preferred providers for a health plan's members.

ActualMeds pharmacists conduct Monthly Medication Reviews (MMRs) for long-term care patients in over 1,000 Skilled Nursing Facilities nationwide and provide data and reports to support Medicare requirements for SNF review and audit. CMS is set to release more stringent coding requirements linked to reimbursement, effective from October 1, 2023. Accurate data at the time of admission will be critical to meeting these new requirements, and ActualMeds is the only provider capable of delivering a complete view of a patient's validated medications from admission to discharge.
